In order to register you have to
fill in 2 forms:
- The personal information’s form (Yellow Form)- Fiche Individuelle de Renseignements,
to be filled and signed by the parents
or the legal tutor of the
child.
- The medical form (White Form)- Fiche Sanitaire de Liaison, to be duly
filled in and signed by the child’s physician.
It is very important to
print out those forms, fill them in, sign them and send them by fax before the
deadline for registration.
The originals would be handed in
personally.
To be provided with the forms:
- 2 recent ID
photos
- A photocopy of the identity card or passport.
It is
highly recommended for the organizers to meet the children during the
registration period.
A third form meant for the “trousseau”
of the child (Blue Form) – Fiche Individuelle de Trousseau,
is to be used along with the child while packing the personal belongings.
( The blue form is to be put inside the suitcase after packing )
